Tesamorelin is a synthetic analog of growth hormone-releasing hormone (GHRH) designed to stimulate endogenous growth hormone (GH) secretion from the anterior pituitary. By binding to the GHRH receptor, tesamorelin enhances the pulsatile release of GH, which in turn increases circulating levels of insulin-like growth factor-1 (IGF-1), promoting lipolysis, lean body mass preservation, and improved metabolic function. Clinically, tesamorelin has been primarily investigated and approved for reducing visceral adipose tissue in patients with HIV-associated lipodystrophy, demonstrating significant decreases in abdominal fat without adversely affecting subcutaneous fat distribution.
