Thymosin Beta-4 (TB-500) is a synthetic peptide analog of the naturally occurring Thymosin β4 protein, which is ubiquitously expressed in mammalian tissues and plays a critical role in tissue regeneration, cell migration, and wound healing. TB-500 primarily functions by upregulating actin polymerization, which facilitates cytoskeletal remodeling, endothelial cell migration, and angiogenesis. Preclinical studies have demonstrated that TB-500 accelerates repair in muscle, tendon, ligament, and dermal tissues, while also exhibiting anti-inflammatory effects through modulation of cytokine expression and suppression of fibrotic pathways. In addition, TB-500 has been shown to promote the expression of vascular endothelial growth factor (VEGF) and other pro-angiogenic factors, enhancing neovascularization in damaged tissues. Pharmacokinetic analyses indicate that TB-500 has a prolonged systemic half-life compared to endogenous Thymosin β4, allowing for sustained biological activity following subcutaneous administration. While robust animal data support its regenerative potential, human clinical evidence remains limited, and ongoing research is required to define optimal dosing, tissue-specific efficacy, and long-term safety, particularly regarding off-target angiogenic effects.
